Your Care Planning Session (60 minutes)

In a focused one-on-one or family session, we dive into the details- what’s happening now, what decisions are on the table, and what needs the most attention. After the call you’ll receive a practical, customized care plan that reflects your family’s values, limitations, and goals.

Moving Forward
(30 minutes)

After you’ve had time to absorb the plan and take a few steps, we’ll reconnect to troubleshoot, clarify next moves, and make sure you feel supported and steady.

Ongoing support

For families looking for continued guidance, evolving strategies, or someone to walk alongside them over time. For those clients, we offer monthly retainer packages with flexible session scheduling and ongoing access to professional support.
